What are your standard charges
for a comprehensive eye examination?
A comprehensive examination for a new patient costs $150.00. A comprehensive
examination for an existing patient costs $130.00.
What types of insurance do you
accept?
We accept all forms of insurance, including government programs,
such as Medicare, Medicaid and Public Aid; vision plan providers,
Vision Service Plan (VSP) and others; and
many private medical insurance plans.
What insurance documentation do
I need to bring with me?
A current copy of your insurance card must be presented when you
arrive to your appointment.
How can I find out if you are
an accepted provider for my private health insurance carrier?
Please contact your insurance company to verify that the Illinois
Eye Institute is an accepted provider by your health insurance carrier.
Will you submit a claim form to
my insurance company or do I have to?
We submit the claim forms for government-sponsored programs, such
as Medicare and Medicaid. You are responsible for submitting claim
forms for any private insurance carriers. IEI will provide you with
an itemized bill that you may submit with your claim form to your
insurance company each time you visit.
Am I responsible for any part
of the payment at the time of my visit?
Except for Medicare, Medicaid and Public Aid, you are responsible
for payment on all services rendered at the time of your visit.
We will provide you with an itemized bill that you may submit along
with your claim form to your insurance company. For eyeglasses,
50 percent of the cost is due upon placing the order with the remainder
due when you pick up your glasses.
